They illustrate a fatal strategic flaw. They treat human oversight as a “check” at the end of a process. In the world of Organizational Strategy for Generative AI, we have a name for this: The Janitor Trap. If you only have a Human-in-the-loop, you’ve already lost.

By the time the human sees the output, the agent has already hallucinated, biased the data, or triggered a tool with unintended consequences. You are just asking a human to mop up an automated mess.

The Shift: Human-BEFORE-the-loop

The next generation of agentic architecture, what we are discussing in advanced circles, is Human-before-the-loop (HB4L). HB4L isn’t about checking the work, it’s about authoring the intent.

Using MCP (Model Context Protocol) as just a “technical bridge” is a waste of a revolution. MCP should be the enforcement layer for HBTL governance. Before the agent acts, the human must have already:

Hard-coded the boundaries into the context protocol. Pre-defined the ethical overrides that the agent cannot “reason” its way out of. Scoped the agency so the “Brain” isn’t a wildcard, but a specialized tool under human command.

The Brutal Reality of Scale:

“In-the-loop” fails at machine speed. A human cannot “oversee” 10,000 transactions per second. If your governance is reactive, it is non-existent.
The danger isn’t that the AI will fail. The danger is that it will succeed at doing exactly what you didn’t tell it to avoid.

When you remove the “Human-before-the-loop,” you aren’t building a smart organization; you’re building a highly efficient engine for reputational suicide.

My Advice to Leaders: If your AI roadmap doesn’t have a clear “Before-the-Loop” architecture, you aren’t ready for agents. You’re just waiting for a disaster that moves too fast for your “Janitors” to clean up.
